Identify Eligible Patients

Review the program eligibility criteria and identify ten (10) patients to include in the DEMONSTRATE Program
Download the Internal Patient Log

Save the Patient Log to your desktop and make note of where the file is saved for easy access

Record the corresponding information in the Patient Log as you are completing the data collection forms.

Save and print the completed Patient Log

Record the eligible patient information. Keep a copy of this document as you will need to reference it when completing the subsequent program phase

Thank you for agreeing to participate in the DEMONSTRATE Program. We look forward to collaborating with you.

The goal of the DEMONSTRATE Program is to assess the current approach to the secondary prevention management of post-MI patients by cardiovascular specialists in Ontario. This program is coordinated by the CHRC and is supported by PendoPharm Canada.
Participants who complete all the required program components will be compensated $2,000.00 by the CHRC.
The program steps are outlined in the left hand menu and are listed in the order in which they should be completed. Click on the "icons" to access the specific step component. We also encourage you to review the Instructional Manuals for a more detailed description of each step and instructions on how to complete the program forms.
